1.1 The Chair welcomed Members to the meeting.
1.2 Apologies were received from Sarah Murphy MS.
2.1 The Committee received evidence from mental health representatives.
2.2 Oliver John, Chair – Royal College Mental Health Expert Advisory Group agreed to write to the Committee regarding the delivery of integrated support to people with long-term conditions who live in nursing homes and care homes.
3.1 The Committee agreed the motion.
4.1 The Committee discussed the evidence received.
4.2 The Committee agreed to write to the panel about the support available for children and young people with chronic conditions as they move from paediatric to adult care.
5.1 The Committee agreed its forward work programme for the summer term.
6.1 The Committee agreed the areas for discussion in the paper.
7.1 The Committee received evidence from the Chief Medical Officer for Wales.
7.2 The Chief Medical Officer for Wales agreed to write to the Committee with details of the protocol developed to address communication problems between primary and secondary care.
8.1 The Committee noted the letter and that another Senedd committee was undertaking a piece of work in this area.
8.2 The Committee noted the letter.
8.3. The Committee noted the letter and agreed to write to the Finance Committee Chair.
